MODULE 4

BOWEL MANAGEMENT

4.1 Introduction

At the start of Module 1, Patricia begins by breaking Bowel management into simple terms for the trainee. She also clarifies which clients would be in need of bowel management assistance and why.

4.3 Digestive System

A second presentation of the digestive system is then conducted, but this time contrasting the digestion of a tetraplegic against that of an able bodied person. This gives context to some of the interventions made around laxatives, softeners and suppositories.

4.5 Bowel Routine: Laxatives Etc

Here, trainees are shown the various interventions made to make the client's feeding and toileting as problem-free as possible.

4.7 Bowel Routine: Main Session

The aim of the main session presentation, is to show the trainee how to set themselves up for success. This includes showing them which materials to have on hand before they start, how to make the client comfortable during the session, and how to keep the environment clean and safe.

4.8.1 Bowel Routine: DRS

Stimulating a client's bowel is a very delicate matter requiring skill and confidence from the trainee. So in this section, we show how to safely and professionally conduct the procedure so that the client is comfortable, relaxed and happy.

4.8.3 Bowel Routine: Clean Up

Keeping the client clean is an essential part of the Bowel Routine. And here, the presenter shows the trainee how to ensure the client is left perfectly clean, before moving on to the next tasks.

4.8.5 Bowel Routine: Skin Check

Here, the presenter calls the trainees attention to the client's body explaining the importance of checking the skin, and showing them what to look for .

4.8.7 Night Bag: Sidenote

Before teaching the trainee how to disconnect a night bag, the presenter will explain the reasoning behind catheterisation and why a paralysed client would need one.

4.8.9 Disconnect Night Bag

Now that the trainee has been introduced to the catheter, the presenter then instructs them on how to disconnect the night bag from the leg bag.

4.2 Food Digestion

Using 3D models, the presenter briefs the carer on the basics of an able bodied person's digestive system including, what happens to food when it's eaten and how it's processed in the digestive system.

4.4 Bowel Routine: Considerations

Here we're careful to share with the trainee the things that are most important to the client around Bowel Management and Bowel Routines. This  is to ensure that they respect and treat each customer with dignity.

4.6 Bowel Routine: Pre-session

With the use of 3D generated mannequins, the presenter shows the trainee what we do in preparation for the Bowel Management main session. As the camera rotates around the figures, your carers will better understand exactly how to carry out the procedure, and therefore be more confident when they do it for real.

4.8 Bowel Routine: Initial Check

Those new to bowel management will appreciate the attention to detail given to the process. This includes the 'Pre-Check' stage of bowel management. Here we show how to insert their finger, how to assess what they find and what to do next. Our 3D mannequins allow us to show the rotation of the finger inside the client further enhancing the clarity of the procedure.

In this part of the presentation, the presenter shares with the trainee how to describe the stool types produced.

Clearing up without contaminating other surfaces is a skill the presenter shares using simple  terms and common sense.

4.8.6 Pressure Sores: Sidenote

This part follows on from the Skin Check section, looking in more detail at pressure sores and using 3D generated imagery to illustrate what they look like and how they develope.

4.8.8 Catheters

In this section, our presenter shows the trainee, how a suprapubic catheter looks when installed, with the camera moving in close to the 3D model and moving along the tube to the leg bag and then the night bag.
